Log in to the client portal with your credentials.

Navigate to the ‘other request’ module on the left sidebar and select ‘Accident Report’
Fill out the form with the injured employee's information.
Under ‘accident detail,’ fill out the information about the accident, including the date and location.
Complete the form and click ‘Submit’
